Who is required to complete the Health Declaration Form?
All persons arriving in Nigeria from international destinations — including Nigerian citizens returning from abroad — are required to complete the Health Declaration Form. This is mandatory under the International Health Regulations (IHR 2005).
When should I submit my Health Declaration?
You should submit your declaration before boarding your flight or vessel to Nigeria, or at the latest, upon arrival at any Nigerian port of entry. We strongly recommend submitting at least 24 hours before your arrival to avoid delays.
What do I need before filling the form?
You will need: (1) your international passport, (2) your flight/vessel booking confirmation with flight/vessel number, (3) a valid phone number, and (4) a valid email address to receive your QR Code and PDF certificate.
How do Port Health Officers verify my declaration?
Upon submission, you receive a PDF with an embedded QR code. Port Health Officers at all entry points have devices capable of scanning this QR code to instantly pull up and verify your declaration on the NCDC Health Declaration System in real time.
Is my personal and health data safe?
Yes. All data is encrypted using industry-standard encryption. The NCDC Health Declaration System is fully compliant with the Nigeria Data Protection Regulation (NDPR). Your data is only accessible to authorised NCDC and Federal Ministry of Health personnel.
What happens if I declare symptoms?
If you declare symptoms, your entry may be flagged for additional health screening at the port of entry by Port Health Officers. This is a standard public health precaution and does not necessarily mean you will be denied entry.
